INSURANCE
We ask that patients obtain any required referrals from their primary care physician prior to their scheduled appointment, and bring a copy to their visit.
Insurance Co-payments are required at time of Service.
Should you require surgery the staff will make every effort to see that your insurance company has been notified and that all precertification requirements have been met. We encourage you to communicate with your scheduler to insure this has been taken care of, if applicable.
You are our best source of information regarding your coverage. Please provide us with up-to-date information regarding all of your insurance policies.
